How to use it:
Please provide all information requested in order to meet the requirements for recertification. REMINDER: Although we do not require supporting documentation with the submission of your recertification form, you should retain appropriate documentation of your continuing education activities for two full years after the end of your recertification cycle in the event of an audit. A recertification audit is conducted after each recertification cycle. If you are randomly selected for the audit, you will be required to provide this documentation for verification of the continuing education credits reported on your recertification form.
